Citizens Bank of West Virginia, as LIENHOLDER/LOSS PAYEE | PO Box 1519, Elkins WV, 26241.
Comprehensive and Collison coverage with deductibles not to exceed $1000.
Policy must match collateral on loan document.
Policy Number, Policy Period, and Company Name MUST be listed.
Effective Date of policy must be day you took possession of vehicle or travel trailer.
