• Citizens Bank of West Virginia, as LIENHOLDER/LOSS PAYEE | PO Box 1519, Elkins WV, 26241.
• Comprehensive and Collison coverage with deductibles not to exceed $1000.
• Policy must match collateral on loan document.
• Policy Number, Policy Period, and Company Name MUST be listed.
• Effective Date of policy must be day you took possession of vehicle or travel trailer.
